Equipment Request

If you or a loved one are interested in requesting equipment for pickup, please select an appointment below along with a date and time to come pick up your equipment. You will also be asked to answer a few questions about your needs and background. All of this information will remain confidential and will not be disclosed to any persons, businesses or organizations. This information is only used within our records at The Dancing Goat DME.

As a reminder, our store front is only open on Tuesdays from 11am - 2pm. You will need to schedule your pickup during this time.
